CONSENT FORM FOR TREATMENT

As Healer(s) associated with HealerLink, we do not medically diagnose or prescribe treatment. Our approach is holistic and focuses on you as a complex, unique being -- mind, body, and spirit -- and we serve as facilitators in your process of healing.

We may explore areas that influence your state of well-being, such as health history, life stressors, your belief systems and attitudes, your family and childhood history, diet, exercise, and how you are in relationship with yourself and others.

Your sharing is always kept confidential. We do, however, discuss clients (without mentioning their names) with professional supervisors or professional peers as local laws permit for the purpose of our continuing professional development.

CLIENT RESPONSIBILITY AGREEMENT

VortexHealing® Divine Energy Healing, Emotional Freedom Techniques (EFT), Unwinding the Fascia Energy, Life Force Healing, Spiritual Awakening, Non-Duality, and other holistic healing modalities and practices (from now on together called as Healer Arts) listed on HealerLink website are very powerful healing arts. Therefore, it is to be expected that various situations can arise from receiving these Healing Arts.

On one hand, certain problems, either physical, mental, emotional, or spiritual, may be alleviated. Sometimes, deep mystical experiences occur, as well as life-changing realizations. On the other hand, suppressed emotions or emotional patterns may receive enough healing energy to be pushed to the surface so that they can be cleared and resolved, and this process may also create various physical symptoms.

Also, release of physical tension in one area of the body may bring to the surface tension that has been stuck in the other areas of the body, which can manifest as various bodily symptoms. When there is deep tension in the body, pain may occur as that tension releases and the body rearranges itself. In homeopathy, it is expected that old and even new symptoms will arise as part of the healing process. Although this doesn't usually happen with these Healing Arts, occasionally it does. It is all part of the healing process.

I agree that I have read and understood the above paragraphs provided by HealerLink and agree that the Practitioner is not responsible for any individual symptoms that may arise as a result of receiving these Healing Arts treatments. I agree to take personal responsibility for whatever physical, emotional, mental, or spiritual symptoms may arise as part of the healing process of receiving Healing Arts treatments, as well as take responsibility for seeking medical treatment when I perceive it is necessary.

I also understand that my Healing Arts Practitioner is not a medical professional and that he or she neither practices medicine nor takes the place of medical treatment, diagnoses, and evaluations, when needed. For those, I consult qualified physicians. I also agree that HealerLink and their Healing Arts Practitioners may keep a record on file of any and all information that I have given them, whether in written, oral, or other form, from now on. Any questions I currently have, have been answered to my satisfaction.

The client confirms that he or she has presented themselves in their own name, in good faith, and for no other reason than obtaining a natural energy treatment. 

I also agree that I am liable for payment for any scheduled appointment unless I give notice of cancellation at least 24 hours beforehand.
